Background:
Derived from the National Pediatric Cardiology Quality Improvement Collaborative registry, the NEONATE risk score predicted freedom from interstage mortality or heart transplant for patients with single ventricle CHD and aortic arch hypoplasia discharged home following Stage 1 palliation.
Objectives:We sought to validate the score in an external, modern cohort.
Methods:This was a retrospective cohort analysis of single ventricle CHD and aortic arch hypoplasia patients enrolled in the National Pediatric Cardiology Quality Improvement Collaborative Phase II registry from 2016 to 2020, who were discharged home after Stage 1 palliation. Points were allocated per the NEONATE score (Norwood type—Norwood/Blalock–Taussig shunt: 3, Hybrid: 12; extracorporeal membrane oxygenation post-op: 9, Opiates at discharge: 6, No Digoxin at discharge: 9, Arch Obstruction on discharge echo: 9, Tricuspid regurgitation ≥ moderate on discharge echo: 12; Extra oxygen plus ≥ moderate tricuspid regurgitation: 28). The composite primary endpoint was interstage mortality or heart transplant.
Results:In total, 1026 patients met inclusion criteria; 61 (6%) met the primary outcome. Interstage mortality occurred in 44 (4.3%) patients at a median of 129 (IQR 62,195) days, and 17 (1.7%) were referred for heart transplant at a 167 (114,199) days of life. The median NEONATE score was 0(0,9) in those who survived to Stage 2 palliation compared to 9(0,15) in those who experienced interstage mortality or heart transplant (p < 0.001). Applying a NEONATE score cut-off of 17 points that separated patients into low- and high-risk groups in the learning cohort provided 91% specificity, negative predictive value of 95%, and overall accuracy of 87% (85.4–89.5%).
Conclusion:In a modern cohort of patients with single ventricle CHD and aortic arch hypoplasia, the NEONATE score remains useful at discharge post-Stage 1 palliation to predict freedom from interstage mortality or heart transplant.

The Fontan Outcomes Network was created to improve outcomes for children and adults with single ventricle CHD living with Fontan circulation. The network mission is to optimise longevity and quality of life by improving physical health, neurodevelopmental outcomes, resilience, and emotional health for these individuals and their families. This manuscript describes the systematic design of this new learning health network, including the initial steps in development of a national, lifespan registry, and pilot testing of data collection forms at 10 congenital heart centres.

As part of an ongoing effort to increase image reproducibility and fidelity in addition to improving cross-instrument consistency, we have proposed using four separate instrument quality tests to augment the ones we have previously reported. These four tests assessed the following areas: (1) objective lens quality, (2) resolution, (3) accuracy of the wavelength information from spectral detectors, and (4) the accuracy and quality of spectral separation algorithms. Data were received from 55 laboratories located in 18 countries. The largest source of errors across all tests was user error which could be subdivided between failure to follow provided protocols and improper use of the microscope. This truly emphasizes the importance of proper rigorous training and diligence in performing confocal microscopy experiments and equipment evaluations. It should be noted that there was no discernible difference in quality between confocal microscope manufactures. These tests, as well as others previously reported, will help assess the quality of confocal microscopy equipment and will provide a means to track equipment performance over time. From 62 to 97% of the data sets sent in passed the various tests demonstrating the usefulness and appropriateness of these tests as part of a larger performance testing regiment.

Marine and terrestial animals have been shown to be particularly abundant in a wildlife community associated with a marine sewer outfall from the City of Vancouver. These same animals are contaminated with high levels of heavy-metals but are apparently protected from their poisonous effects by the production of a protein known as metallothionein. The amount of metallothionein and heavy-metal loading appears to depend primarily on the degree of pollution and secondly on the species of animal and its position in the food-web.

Summary
Advances in the technology of communication devices and communication training have greatly expanded the proportion of affected individuals who can reacquire the ability to communicate. The growing body of knowledge and clinical experience related to supporting communication by means of assistive devices and techniques is known as augmentative and alternative communication (AAC) or augmentative communication. There are many kinds of actions that can be sensed and converted into the equivalent of a switch closure or a computer keystroke. Just as devices can be categorized, device-users are often considered in terms of the disability that is at the root of their need for communication support. This chapter points out considerations that are often of special concern for particular disability groups. Communication devices could contribute considerably more, and may do so in the near future, in order to enable the citizens to express their full cognitive, social and emotional potential.

The interdisciplinary approach to the study of urban history has considerable potential for the examination of communites of manageable size. Yet, as Professor Dyos has pointed out, it is by no means clear that the proliferation of more and more case studies is a particularly useful exercise unless communities in question contain an element of typicality and are informed by the broader social and historical tendencies relating to urbanization as a major force of social change. “We know so little yet about a number of obvious things,” he once wrote. “Small towns that never grew big nor got stuck in amber as a neighbour did tend to remain unseen. There are too few studies of such towns.” (H. J. Dyos (ed.), The Study of Urban History [1968], 38.) Our study, of Kendal in Westmorland, seems well justified.
